FP69

The FP69 report provides a simple method of listing patients with an outstanding FP69 flag by earliest expiry date with a primary focus on those patients that have not answered correspondence.

It also presents certain data to help the practice complete the outstanding FP69 flag without having to trawl through each patient record,for example, have they been seen recently or be seen shortly or why they don't answer correspondence, all are valid reasons that can be reported back to the HA:

  • When patient joined practice
  • Birthplace
  • Number of face-to-face consultations (listed in workbook) in last 6 months
  • Number of future appointments
  • Latest clinical term for how patient information should be received from practice (read codes listed in workbook)
  • Latest ethnicity clinical term (9i%, 9S%, 9T%, 9t%)
  • Latest religion clinical term (135%, 13y%, 13z%)
  • Latest interpreter clinical term (1a01%, 9NU%, 9Nm%, 9Nn%, 9NgzL%, 9NgzM%, 9NgzN%)
  • Latest language spoken clinical term (13l%, 13u%, 13w%, 13ZG%, 13ZL%, 13ZM, 13ZP)
  • Latest language Read clinical term (13n%, 13ZA1%, 13ZG0%)
  • Latest LD clinical term (E3%, Eu7%, Eu814% - Eu818z%, Eu81z%, 918e%)
  • Latest MH clinical term (E10%, E110%, E111%, E1124%, E1134%, E114% - E117zz%, E11y%, E11z%, E11z0%, E11zz%, E12%, E13%, E2122%, Eu2%, Eu30%, Eu31%, Eu323%, Eu328%, Eu333%, Eu32A%, Eu329%' ) excluding E11y2%, E135%
